Does anything change in the stored text?

No. The links appear when the page is served. Switch the plugin off and the text is exactly as before.

Should I set nofollow?

For internal links usually not — they are meant to pass weight on. The switch is there for special cases.

What can the template do?

It decides the markup around the link text, an extra icon for instance. The link text itself stays the word that was found.
